Nursing, Radiology and Laboratory
Educational Loan Program

INTRODUCTION:

Stewart Memorial Community Hospital supports and encourages the Nursing, Radiology and Laboratory professions. Stewart Memorial Community Hospital proposes special assistance to individuals who would like to become Registered Nurses, Radiologic Technicians, or Medical Laboratory Technicians. The program is intended only for an Associate Degree (A.D.). This program is intended for those current employees who desire changing positions, or those individuals who are not employed by Stewart Memorial Community Hospital , but plan on employment after achieving the appropriate education and degree.

GUIDELINES:

* The Educational Loan Program is available for individuals interested in a Registered Nursing, Radiologic Technician and Medical Laboratory Technician position. Applications must be made to the Director of Staff Development. Applicant acceptance will depend on information provided in the application form, personal interview, and the availability of budgeted monies for the program.

* The Vice President and the manager of the department involved in the field of study chosen will review each applicant. Consideration will be given to the projected need for personnel and the probability of a successful relationship.

* Stewart Memorial Community Hospital will make an amount of money, equal to one years tuition, not to exceed the tuition required by Iowa Central Community College, Fort Dodge , Iowa , available to the applicant. The money will be available for the necessary two years of schooling required to become a Registered Nurse, Radiologic Technician or Medical Laboratory Technician.

* Normally, the applicant, of a bill to Stewart Memorial Community Hospital , will pay the tuition directly to the educational institution upon presentation. However, an applicant may request Stewart Memorial pay them directly if the applicant can verify that the educational institution has been paid.
* If the applicant takes a full or part time position at Stewart Memorial Community Hospital following graduation, the hospital will forgive one year of financial assistance for each year worked. Loan forgiveness will be pro rated if an employee leaves SMCH before working two years.

* If a position at Stewart Memorial Community Hospital is not available within six months after the applicant has graduated, or if after graduation a work schedule cannot be mutually agreed upon between the applicant/graduate and Stewart Memorial Community Hospital, or if the applicant desires not to work at Stewart Memorial Community Hospital upon graduation, the full balance of the loan to the applicant, including all previous years, is due and payable immediately. If payment is not made immediately, a 15% annual interest rate will be applied until full payment is made. Stewart Memorial Community Hospital Administration has the discretion as to enforcement upon the occurrence of one or more of the foregoing scenarios.

* If the applicant's degree requires that they pass a State, Federal, or Association Board or Examination, the applicant will have two attempts after the initial attempt to pass the appropriate Board or Examination. If the applicant fails to pass the Board or Examination the third time, the full balance of the loan to the applicant, including all previous years, is due and payable immediately. If payment is not made immediately, a 15% annual interest rate will be applied until full payment is made. If the applicant is an employee at Stewart Memorial Community Hospital , said employee may request that the loan balance be paid back to the hospital on a monthly basis, not to exceed 12 months. No interest will be charged conditioned on the fact that the applicant maintains employment at Stewart Memorial Community Hospital .

* If the applicant fails to complete the required courses or assignments and/or fails to earn the degree, the full balance of the loan to the applicant, including all previous years, is due and payable immediately. If payment is not made immediately, a 15% annual interest rate will be applied until full payment is made. If the applicant is an employee of Stewart Memorial Community Hospital , said employee may pay back the hospital on a monthly basis, not to exceed 12 months. No interest will be charged conditioned on the fact that the applicant maintains employment at Stewart Memorial Community Hospital .
