Social Services

Stewart Memorial Community Hospital's Social Services Department assists hospitalized patients and their families with discharge planning, connecting patients to community resources such as Link to Life, HomeCare, Meals on Wheels and providing supportive counseling. Our social worker case manages homecare patients on the Elderly Waiver and provides psychosocial supportive care to Hospice patients and their families.
