Obstetrics, Pediatrics, and Adult Medicine Provider Susan Hornback, DO, is a board-certified family physician at the Lake City Clinic. She began her medical career as a CMA, eventually becoming an RN, and later attending Des Moines University’s School of Osteopathic Medicine. Dr. Hornback completed her medical residency at Iowa Lutheran Family Medicine in Des Moines and became a member of the American Academy of Family Physicians and the American Medical Association. She enjoys spending her free time with her family.
