What is a chronic condition?
50% of US adults have one or more chronic health conditions, and one in four adults have two or more chronic health conditions.
Common chronic conditions include:

- Kidney disease
- COPD
- Anxiety

Who qualifies for our Care Management program?
One or more chronic conditions are required to qualify.
Please note: the usual cost-sharing rules apply to these services, so patients may be responsible for the balance of coinsurance/copay as contracted with their personal insurance company.
