Care Management

Care Management is a program designed to provide 24/7 care coordination services to patients. Patients will gain a team of dedicated health care professionals who will help them plan for improved health and assist with staying on track. Our staff help patients work towards their health and quality of life goals. Better care management can help patients avoid unnecessary health events such as trips to the emergency department or worsening health conditions.

Coordinated care means patients will get personal attention and help from a health coach who is knowledgeable about their current and past medical history. They will help answer any health related questions, help patients follow through with doctor’s instructions, help to avoid potentially dangerous drug interactions, ensure patients know about and get access to the care they need, and help to locate resources that can improve overall health and lifestyle. Regular check-ins from one of our certified health coaches will improve care coordination as well as increase collaboration between the patient and their primary care provider. 

Patients who have one or more chronic conditions may qualify for care management services. Chronic conditions include, but are not limited to:

  • high blood pressure
  • COPD
  • diabetes
  • heart failure
  • kidney disease
  • arthritis
  • anxiety
  • depression
  • heart disease

Care management services could help connect the dots for these patients, so that they can spend more time doing what they love. If you or someone you know is interested in being part of our care management program simply talk with your healthcare provider. He/She will contact the care management department with a referral and once the patient’s medical record has been reviewed a health coach will be in contact with you. Care management services can help deliver the coordinated care that patients not only need, but care that they deserve. 

Care management services are covered by Medicare, Medicaid, and most commercial insurance companies. Please note that usual cost-sharing rules apply to these services, so patients may be responsible for the balance of coinsurance/copay as contracted with their personal insurance company.
